How to login to the Patient Portal
If you are a patient of Valley Endocrine Associates, you can access their Patient Portal online. The Patient Portal is an online tool that provides patients with secure access to their medical records, appointment scheduling, and more. To access the Patient Portal, please follow these steps:
1. Click the "Patients" tab on the main website.
2. Enter your patient identification number (PIN) in the "Enter your patient identification number (PIN)" field and click the "Login" button.
3. Enter your email address in the "Email address" field and click the "Login" button.
4. Click the "My Medical Records" button to view your medical records. You can also request copies of your medical records by clicking on the "Request Medical Records" link on the My Medical Records page.
5. Click the "Appointments" button to view your current appointments and make changes to your appointments or cancellations. You can also book new appointments by clicking on the "Book Appointment" link on the Appointments page.
6. Click the "Message Boards" button to post messages or ask questions about health care topics related to Valley Endocrine Associates
Changes to the Patient Portal
Changes to the Patient Portal are now available. New features include the ability to add notes and photos, as well as personalize your profile. To access these new features, follow these steps:
1. Log in to the Patient Portal using your patient credentials.
2. Click on My Profile in the top left corner of the screen.
3. Click on Notes and Photos in the sidebar on the right.
4. Add notes and photos as you see fit, and then click Save.
5. If you would like to personalize your profile page, click on Edit Profile in the top right corner of the screen. You can update your name, email address, and contact information, as well as select which medical groups you would like to be part of (if applicable).
Finding your health history

Appointments
Valley Endocrine Associates Patient Portal offers convenient online appointments booking. To login, click on the "Login" link in the upper right corner of the homepage. You will be prompted to enter your username and password. Once you have logged in, you will see the "Appointments" tab on the left side of the page. Click on the "Appointments" tab to view all of your appointments. You can also search for an appointment by name or date. You can cancel or reschedule an appointment by clicking on the "Cancel Appointment" or "Reschedule Appointment" links, respectively. What happens if I can't make an appointment?
If you are unable to make an appointment, please call their office at (855) 489-2345 and ask to be put on the waiting list. They will attempt to see you within a month.

Communicating with your doctor
If you are new to Valley Endocrine Associates, or have forgotten your username or password, please follow these instructions:
1. Log in to the Patient Portal. If you have not yet registered for an account, you will need to do so first.
2. Click on “Forgot your username or password?” on the left-hand side of the screen.
3. Enter your email address and click “Forgot your password?”
4. If you have forgotten your username, enter your email address again and click “Forgot your username?”
5. If you have forgotten your password, enter your email address again and click “Forgot your password?”
6. You will receive an email with instructions on how to reset your password. Please follow the instructions in the email. Appendix A: What if I need urgent care?
If you need urgent care, please call 9-1-1. If you are in pain and cannot wait for a doctor to see you, the ER at Valley Endocrine Associates is open 24 hours a day.
If you have an emergency and cannot wait for a doctor appointment, please go to the ER immediately. Valley Endocrine Associates will schedule you for an appointment as soon as possible.

Appendix B: FAQs for patients
Valley Endocrine Associates Patient Portal how to login
If you have been redirected to their Patient Portal from another website and do not remember your user name and password, please follow these instructions.
First, click on the link that says "Log In." This will take you to a page where you can enter your username and password. Please note that your username is the same as your email address. If you cannot remember your password, please contact them at 415-974-1077 for assistance.
Once you have logged in, you can browse through the various sections of their Patient Portal. The main sections are "My Profile" and "Medical History." In "My Profile," you can update or add information about yourself such as your contact information and medical history. You can also manage your health records by clicking on the blue "My Health Record" button. Finally, in "Medical History," you can view all of the medical procedures that we have performed on you, as well as their results.
Appendix C
Valley Endocrine Associates Patient Portal users can access their account by following these steps:
