Welcome to the Patient Portal of Pueblo Community Health Center! This site is designed to help you manage your health care and keep track of your medical records. Whether you're a new patient or have been visiting us for years, this site can help you stay organized and connected to your health care.
In order to login and access their Patient Portal, please follow these instructions:
1. Enter your full name into the "Login" field on the main page.
2. Click the "Log In" button to enter your user name and password.
3. If you are a new patient, please enter your contact information in the "New Patient Information" field and click the "Create Account" button. You will then be redirected to their secure registration page where you will need to complete additional information.
4. Once you have submitted all of the necessary registration information, you will be able to view your account information and medical records by clicking on the corresponding links on the main page. How to Login to the Patient Portal
If you are new to their Patient Portal, please read the following instructions before you try to login. If you have already logged in, please follow the instructions below for updating your login information.
To log in to the Patient Portal:
1. Click on "Patient Portal" in the main menu of their website.
2. In the "Patient Portal" window, click on "Login."
3. Enter your user name and password and click on "Log In."
4. If you are a new patient or have not registered with them yet, you will be prompted to create a patient account. After you have created your account, please enter your registered email address and password in the appropriate fields and click on "Next."
5. You will now be prompted to choose a health condition or service that you would like to view information about. After selecting a health condition or service, you will be able to access detailed information about your care by clicking on the links provided under each category.

How to Access your Medical Records
If you have ever had to go to the hospital or visit a doctor, you have probably used their patient portal. A patient portal is a website that allows patients to easily access their medical records, order prescriptions, and track their health trends.
To access your Pueblo Community Health Center patient portal, follow these simple steps:
1. Go to www.pchc.org and click on the “Patients” tab on the left-hand side of the screen.
2. You will see a list of doctors who are affiliated with PCHC.
3. Click on the doctor’s name to open his/her patient portal page.
4. On the doctor’s patient portal page, you will see a “My Records” tab at the top of the page.
5. Click on the “My Records” tab to open your medical records page.
6. On your medical records page, you will see a list of all of your appointments and treatments that have taken place at PCHC since you last visited their clinic or hospital. You can also view your lab results and pharmacy information here!
How to Request an Appointment
Pueblo Community Health Center offers a patient portal that allows patients to request appointments, view their medical history, and more. To use the patient portal, patients must first login. Here are instructions on how to login:
1. Go to www.pchc.org and click on "Patient Portal."
2. On the Patient Portal home page, click on "Login."
3. Type in your email address and password into the appropriate fields and click on "Log In."
4. On the "Welcome" page, click on "Appointments."
5. Under "Appointments Requested," select the date and time of the appointment you would like to schedule. You can also select a type of appointment (e.g., walk-in, clinic). Click on the blue button next to the appointment time to add it to your calendar. If you have multiple appointments scheduled for that day, you can select which one you want to book first by clicking on the checkbox next to that appointment's date and time. You can also add notes about the appointment if you have any questions about what will happen during that visit. Click on "Next."
