How to login to the Patient Portal
If you have forgotten your password, or need to reset it, please follow these steps:
1. Click on the link "Forgot Password?" located in the main menu of the Patient Portal.
2. Enter your email address and click on "Reset Password". If this does not work, please contact them at 1-866-272-6873.
3. A new password will be sent to your email address. Please remember to keep this password confidential.
4. To login to the Patient Portal, enter your new password in the login form and click on "Login".
How to update your personal information
If you have an account with the Patient Portal, you can update your personal information by logging in to your account and clicking on the "Update Profile" link on the Home Page. You will need to enter your name, email address, and password to complete the update process.
How to report a problem
If you experience a problem using the Patient Portal, please report it to us. Here are instructions on how to do so:
1. Log into the Patient Portal using your username and password.
2. Click on “Report a Problem” in the main menu.
3. Enter as much information as possible about your problem, including:

If you want to sign out of the Patient Portal, follow these steps:
1. Click the “Log Out” link on the left side of the homepage.
2. Type your email address in the “Email” field and click “Log In.” You will be logged back in to the Patient Portal.
