How to View Your Medication History
If you are a Hawthorn Medical patient, you can view your medication history on the portal. To login to the portal, follow these steps:
1. Go to www.hawthornmedical.com and click on “My Account” in the top right corner of the screen.
2. On the My Account page, click on “My Medications” in the left navigation menu.
3. On the My Medications page, select “Patient Portal” from the drop-down menu under “Viewing Your Medication History.”
4. Enter your password and click on “Login.”
5. You will now be taken to the Patient Portal page. Here, you can view your current medications and any past prescriptions that you have received from Hawthorn Medical.
How to Request a Copy of Your Health Records
If you are a Hawthorn Medical patient and would like a copy of your health records, please follow these instructions:
To request a copy of your health records, please complete the online form at www.hawthornmedical.com/patient-login and provide your name, address, and phone number. Your request will be processed within five business days. You will receive a confirmation email with a link to download your health records.

Click on the "My Account" button in the top right corner of the patient portal screen. 2. In the My Account area, click on the "Login" button to log in to your account. 3. Enter your user name and password in the appropriate fields and click on the "Log In" button to log in to your account. 4. You will now be able to access all of your personal information and files associated with your patient portal account.
