2. In the upper right corner of the main screen, click on the ?Contact Us? link.
3. In the ?Contact Us? box, enter your question in the text field and click on the ?Send Message? button.
4. A confirmation message will be sent to you confirming that your message was sent to the patient portal and asking for your user name andPassword for logging in. If you do not receive this message, please check your spam folder or contact them at [email protected] for assistance.
5. Enter your user name and Password in the corresponding fields on the next page, and click on the login button to log in to the patient portal.
