Here's how you can sign up for the portal and get started using it:
First, go to www.charlestonhealth.org and login with your username and password.
Next, click on "Patient Portal" in the main menu bar at the top of the page.
On the Patient Portal home page, click on "Create an Account."
Enter your name, email address, and password in the appropriate fields and click on "Create Account."
You'll now be taken to a confirmation page where you can click on the link in the email that was sent to you to activate your account. Once you've activated your account, you'll be able to access all of the features of the patient portal!

How to Request a Review of Your medical Record
If you need to request a review of your medical record, you can do so through the Charleston Ent Patient Portal. The portal is located at www.charlestonent.org and requires you to input your contact information, including your email address and medical insurance information. Once you have logged in, you will be able to access the Request a Review form.
To begin the review process, you will need to provide some basic information about your health, including your name, date of birth, and medical history. You will also be required to provide a copy of your medical record or scan of it. After you have completed the form, please allow up to 14 business days for a response.
