1. Click the "Forgot password?" link in the login screen.

If you have not changed your login information since last logging in, simply enter your username and the password that you used when you last logged in. 2. On the Patient Portal page, you will see a field titled “Create Account” or “Sign In.” If you are a new patient, enter your first and last name into this field, and then click on the “Create Account” button.
3. After you have created your account, you will be redirected to the Patient Portal home page where you can log in using your username and password. If you have forgotten your username or password, please contact their office at 610-965-8111 for assistance.
How to payment for your visit
To make a payment for your visit at Chalfont Family Practice, please follow these simple steps:
1. Log in to their Patient Portal. If you are not currently registered, click on "Register Now" in the upper right hand corner of the main screen and follow the instructions.
2. Click on "Payments" on the left side of the screen. This will take you to a screen where you can make your payment. You may use either Visa or Mastercard. We accept American Express, Discover, and JCB cards as well. Please note that we do not accept checks at this time.
3. Once you have made your payment, click on "Submit Payment." This will take you back to the main Patient Portal screen where you can view your recent payments and reviews.

If you have a patient portal account, you can view your health history by clicking on the "History" tab on the main page. The history tab includes a list of all of your appointments, tests, and medications. You can also click on any individual appointment to view more detailed information about that appointment.

Chalfont Family Practice has a patient portal that allows patients to electronically request information, see their latest lab results, and make appointments. If a patient believes that their query is urgent, they may flag the request as such by clicking on the “Flag as Urgent” button located at the top of the page. If a request is flagged as urgent, Chalfont Family Practice will move it to the top of the queue and attempt to contact the patient as soon as possible.
