How to get started with your care plan
If you have not already done so, please create an account on the Agewell Provider Portal. This is where you will enter all of your personal information in order to manage your care plan. After logging in, click on the My Plans tab and you will be able to see all of your current care plans.
To add a new care plan, click on the blue Add New Plan button near the top of the page. This will take you to the Add New Care Plan form. On this form, you will need to provide your name, address, and contact information as well as the name and contact information of the patient(s) who will be enrolled in your care plan. You can also choose to add a description of your care plan. Once you have completed this form, click on the blue Submit button at the bottom of the page.
To edit an existing care plan, click on the blue Edit Plan button near the top of the page. This will take you to the Edit Care Plan form. On this form, you will need to provide your name, address, and contact information as well as the name and contact information of the patient(s) who will be enrolled in your care
How to pay for your care plan
You can pay for your care plan by clicking the "pay now" button on the Agewell Provider Portal. You will be prompted to enter your credit card information. Once you have submitted your payment, you will be redirected to a confirmation page.
How to make changes to your care plan
If you have any questions about your care plan or would like to make changes, you can login to the Agewell Provider Portal and access your account.
