How to report a safety concern
If you have a safety concern about Vold Vision, please login to their patient portal and submit your report. To login, please enter your clinic email address below. Once logged in, please click on the "Report a safety concern" link in the header of the patient portal. Please provide as much information as possible in your report, including:

-The date and time of the incident
-A description of what happened
