The Virginia Spine Institute Patient Portal is a online tool that allows registered patients to access their medical records, view appointment status and make payments. If you are a registered patient and have not yet logged in, please follow the steps below.
To login to the Patient Portal, you will need your patient ID number and password. Your patient ID number is printed on your medical record, or you can find it on this website under "My Account." You will also need your password to log in. To create a new password, click "Forgot Password?" on the login screen and enter your name and email address. A new password will be sent to your email address. If you do not receive a new password email within a few minutes, please check your spam folder.

4. On the left side of the window, under My Account, you will see your current account information, including your User ID and Password. Under My Records, you will see a list of all of your visits to Virginia Spine Institute and any tests or procedures that we have performed on you. You can also view detailed information about each appointment that you have made at Virginia Spine Institute. Under Resources, you will find links to other useful resources such as patient FAQs and publications about spine
How to make a request for a copy of your health records
To make a request for a copy of your health records from the Virginia Spine Institute, please visit their Patient Portal and click on "Make a Request."
Once you have logged in, you will be able to select the "Health Records Request" form. You will need to provide your name, date of birth, social security number, and insurance information. Once you have completed the form, click on the "Submit" button. A representative from their office will contact you to schedule a time for your health records to be delivered.
How to report an adverse event
If you experience an adverse event while receiving care at the Virginia Spine Institute, please report it to the clinic immediately. This is important so that we can investigate and take any necessary corrective measures. You can use their patient portal to report your event. The following steps will help you complete the reporting process:
