2. On the login screen, enter your email address and password. If you have not registered with the Village Medical Center Patient Portal, you will be prompted to do so now.
3. Once you have logged in, you will be able to view your account details and individual medical records. You can also schedule appointments and access other medical information through the Village Medical Center Patient Portal.

If you have forgotten your password, or need to make changes to your account information, follow these steps:
1. Log in to the Patient Portal.
2. Click the My Account link on the left-hand side toolbar.
3. Enter your email address and password in the appropriate fields and click the Log In button.
4. If you have more than one account at Village Medical Center, select the account you want to make changes to from the drop-down list on the My Account page. If you don't have an account yet, click the Create An Account link on the right-hand side of this page and follow the instructions.
How to report an issue with the Village Medical Center Patient Portal

1.Go to patient portal homepage and click on "Report an issue" link in the top right corner.
2.Enter your information in the form and click on "Submit report" button.
