How to select a health care provider
If you have a Us Family Health Plan, you can use the Provider Portal to select a health care provider.
To access the Provider Portal, go to usfamilyhealthplan.com and sign in.
On the home page, click the blue link labeled “Provider Portal”.
The Provider Portal will open in a new window.
Select the “Provider Profile” tab at the top of the window.
Click on the “Add Your Provider” button.
Enter your provider’s name, contact information, and website address in the appropriate fields.
Once you have entered all of the information, click on “Submit”.
Your provider will be added to the Provider Profile page and you will be able to log in to your account using your provider’s login information.
