One of the benefits of having a patient portal is that it allows you to more easily track your patients' health information. In this Urology Group Patient Portal tutorial, we will show you how to login and access your patient data.
How to login to the Urology Group Patient Portal
If you are a new patient, please follow these steps to create an account and login:
1. Click on the "Log In" button at the top of the page. Enter your email address and password into the appropriate fields. A confirmation message will be sent to your email address.
2. If you have already logged in, please click on "My Account" in the top right corner of the page and enter your email address and password again. If you have forgotten your password, please click on "Forgotten Password?" in the top right corner of the page and enter your email address and new password. A confirmation message will be sent to your email address.
3. If you are a registered patient, please click on "My Profile" in the top right corner of the page and enter your full name, date of birth, contact information (telephone number, fax number, etc.), medications currently being taken, appointment dates for current and upcoming appointments, surgery dates for upcoming surgeries, summary of each surgery performed in past 6 months and comments about your experience with their clinic so far. Your profile will also include links to social media sites where you can share photos or updates about
Using the Patient Portal
Patients can use the Patient Portal to access their medical records, current medications, health information, and more. To login, patients will need to create a login ID and password. They can then use these details to access their account:
To create a login ID, patients will need to click on the "My Account" link on the homepage of the Patient Portal. This will take them to the "My Account" page. On this page, they will need to click on the "Create Login ID" button.
To create a password, patients will need to enter their name and email address in the "Login ID" and "Password" fields, respectively. They should then click on the "Create Password" button to create their password. Patients should keep their login ID and password confidential. If they lose either of these details, they will not be able to log in to the Patient Portal.
Managing Your Medical Records
If you are a patient at the Urology Group, you can use their patient portal to manage your medical records. The Patient Portal is a secure website where you can view your health information, post updates and messages, and track your medical appointments. You can also print or download your medical records. To login to the Patient Portal, please follow these steps:
1. Click on the "Patient Portal" link on the left side of this page.
2. Enter your email address and password in the appropriate fields, and click "Log In."
3. You will be taken to the main Patient Portal screen. Click on "My Medical Records" to open your medical records.
Connecting with Doctors and Nurses
If you're a patient of the Urology Group, you can connect with your doctor and nurses through their Patient Portal. The Patient Portal is a secure website that allows patients to access their medical records, ask questions, and book appointments. To login to the Patient Portal, follow these steps:
1. Go to www.urologygroup.com
2. Sign in using your username and password
3. On the left-hand side of the website, click on "Patient Portal"
4. Enter your patient number and password (which you received when you registered with the Urology Group) in the appropriate fields, and click on "Login"
5. You'll be redirected to the Patient Portal home page. From here, you can access all of your records, ask questions, and book appointments.
Appointments and Follow-Up
If you have any questions about your appointments or follow-up visits, please feel free to contact their office by calling (858) 522-7000 or by logging in to their patient portal. The patient portal is a secure online system that allows you to manage your medical records, communicate with their office staff, and review your medical history. To access the patient portal, visit their website at www.urologygroup.com and click on the "Patient Portal" link on the left hand side of the page.
Discharged from the hospital
Congratulations on your discharge from the hospital! Below we have outlined the steps you need to take in order to login to the Urology Group Patient Portal.

Step 1: Log into your account on the Urology Group Patient Portal using your login credentials (username and password). If you have forgotten your login credentials, please visit their help section and submit a ticket for assistance.
You will be prompted to enter your hospital ID number when first logging in to the portal. This number is located on your hospital discharge paperwork or on your medical record if you were admitted via an ambulance.
Step 2: Once you have logged in, click on the "My Profile" tab located in the top left corner of the screen. You will then be able to see all of your current appointments and treatments.
