If you are a University of Chicago patient and need to login to your patient portal, follow these steps:
1. Go to the University of Chicago website and type “patient portal” into the search bar.
2. Click on the “Patient Portal” link that appears on the page.
3. On the Patient Portal home screen, click on “My Account” in the top left corner.
4. Enter your patient number (found on your ID card or hospital bracelet) into the “Patient Number” field, and click on the “Next” button.
5. Enter your email address into the “Email Address” field, and click on the “Next” button.
6. Enter your password into the “Password” field, and click on the “Next” button.
7. You will now be taken to a screen where you can review your account information and make changes if needed. Click on the “Save Changes” button at the bottom of this screen to save your changes and return to the main Patient Portal home screen.
How to login to the University of Chicago Patient Portal
If you are a University of Chicago patient and have had surgery at the hospital or one of their clinics, you can log in to the patient portal to view your health information and medical records. To login, follow these steps:
1. Go to www.uofchicago.edu/patients/portal/.
2. Click the "Log In" button in the upper right corner of the screen.
3. Enter your healthcare login information, and then click "Log In."
4. You will be taken to a screen that displays your account information and health records.
How to find your medical record
If you are a patient at the University of Chicago, you can find your medical record on the Patient Portal. You can login to the Patient Portal by clicking here. Once you are logged in, you will be able to view all of your records within the Patient Portal.
How to update your contact information
Logging In:
To update your contact information, log in to the University of Chicago Patient Portal. To do this, you will need your login credentials and your patient number.
To retrieve your login credentials, click on the "Forgot Your Login Credentials?" link on the main menu of the Patient Portal. This will open a new window asking for your name and email address. Once you have entered these items, click on the "Submit" button to obtain your login credentials.
Once you have retrieved your login credentials, enter them into the "Login" field on the main page of the Patient Portal and click on the "Log In" button. You will then be directed to a page that displays all of your health records and contact information. To update any information on this page, simply click on the appropriate link and enter the new information into the fields provided.
How to file a complaint
If you have a complaint about your experience with the University of Chicago Patient Portal, you can file a complaint online. To file a complaint, follow these steps:
1. Go to the Patient Portal website and sign in.
2. Click on “My Health” in the top right corner of the screen.
3. On the left side of the My Health page, click on “Complaints.”
4. On the Complaints page, click on “File a complaint now.”
5. Fill out the form and click on “Submit complaint.”
6. A confirmation message will appear saying that your complaint has been submitted and will be reviewed within 48 hours.
