How to login to the Patient Portal

If you have forgotten your user name or password, please click on the "Forgot Your Password?" link located at the bottom of every page. After you enter your user name and password, you will be redirected to the login screen where you can create a new account or log in with an existing account.

How to find information about your health care history and medications
If you're new to the United Health Center Patient Portal, or need help logging in, follow these instructions.
1. Open the Patient Portal at www.uhc.com.
2. In the top left corner, select "My Profile."
3. Enter your first and last name in the fields next to "First Name" and "Middle Name," respectively, and click "Continue."
4. On the next page, under "Personal Information," click on "Health Care History."
This will open a page that lists all of your health care visits and medications, as well as any lab tests or scans you've had done. You can also see information about any chronic illnesses you have, such as diabetes or asthma.
5. To access your medication history, click on "Medications." This will open a list of all of your prescriptions and over-the-counter drugs, as well as their dosages and dates of expiration.

If you are a family member of a patient, please enter your patient's login information below and click on 'Add Family Member.' You will be prompted to create a password and then will be able to log in to the patient's account.
