Triad Patient Portal is a new online health system that was recently launched in the state of Oregon. Triad Patient Portal is designed to help patients better manage their health and care by streamlining the process of accessing treatment records, medications, and other information. Triad Patient Portal How to Login
If you are new to the TRIAD patient portal, or have not logged in for a while, please follow these instructions.
To log in to your Triad Patient Portal:
1. Go to www.triadhealth.org and click on the “My Triad” link in the top left corner of the homepage. This will take you to the My Triad page.
2. Underneath “My Triad Profile”, click on the “Login” button.
3. Enter your User ID and password and click on the “Log In” button. If you have forgotten your User ID or password, please contact them at [email protected] and we will assist you in resetting them.
Triad Patient Portal How to Download Documents
If you have a username and password, enter them below. If not, you can create an account by clicking the "Create Account" button.
If you don't have a username or password, click the "Create Account" button to create an account and then enter your email address and password.
After logging in, you will be taken to the My Triad page. From here, you can access your account information, download documents, and submit comments.:
First, click on the My Triad link in the upper-right corner of the page. This will take you to your My Triad page. Here, you will see a list of all of your triad accounts. If you don't have a username or password, click on the Create Account button to create an account and enter your email address and password.
Once you have logged in, click on the Documents link on the left side of your My Triad page. This will take you to a list of all of the documents that are available in your triad account. The documents are grouped by topic (for example, Clinical Practice Guidelines). You can download any document that is available in your

If you forget your password, or if you would like to change it, follow these steps:
1. Click on the link in the header of this page titled "Forgot Password?".
2. Enter your email address in the form on the next page and click on "Create New Password".
3. You will receive an email with a link to reset your password. Follow the instructions in the email to reset your password.

Triad Patient Portal What is a Profile?
A Profile is a place where you can store information about yourself, including your name, email address, and password. You can also add other personal information, such as your doctor's office name and telephone number.
To create a new profile, click on the "Create Profile" link in the top left corner of the Triad Patient Portal. Enter your name and email address in the appropriate fields, and then click on the "Create Profile" button.
If you have already registered with Triad, your profile will be automatically populated with the information that we have on file for you. If you have not registered with Triad yet, you will need to enter your name and email address first, and then click on the "Create Profile" button to generate a new password.
To update any of your personal information in your profile, simply click on the "Edit Profile" link next to that information.
Once you have created or updated your profile, you can access it at any time by clicking on the "My Profile" link in the top right corner of the Triad Patient Portal. This page will let you see all of the information that is currently stored in your profile, as well as any updates that have been

If you are having an issue logging into the Triad Patient Portal, please follow these steps:
1.Open the Triad Patient Portal and sign in.
2.Click on the "Help" tab in the top right corner of the page.
3.On the "Help" tab, click on "Report an Issue."
4.Enter your information into the form and click on "Submit."
