How to access the Thompson Family Clinic Patient Portal

4. On the left side of the screen, click on My Health History to open the My Health History window.
5. Click on the Account link in My Health History, and then click on Change Password in the pop-up window. Enter your new password twice, and then click OK to close the Change Password window.
6. Click on My Health History again, and then click on Clinics in the left sidebar to view all of their clinics. Click on the clinic you want to view your health data for, and then click on Login in the top right corner of the screen.
7. Enter your patient ID number or name into the text box, and then click Login. You'll be taken to a page where you can view your medical records from that clinic.

How to update your contact information
If you have any changes to your contact information, such as a new phone number or email address, you can update it on the Thompson Family Clinic Patient Portal. 2. Click on "My Medical Records" in the left-hand column of the Patient Portal screen and then click on "Request a Copy."
3. In the "Request for Copy of Medical Record" form, please provide your name, date of birth, and physical address. You may also include information about any medical conditions you have, including any treatments you have received from the Thompson Family Clinic.

How to report a health concern
If you are experiencing a health concern, you can login to the Thompson Family Clinic Patient Portal and report your concern. To login to the Patient Portal, follow these steps:
1. Go to http://www.thompsonfamilyclinic.com/patients/patient-portal/.
2. Click the “Log In” button in the upper left corner of the page.
3. Enter your patient ID number and password.
4. Click the “Log In” button in the upper right corner of the screen.
5. Select “Patient Profile” from the menu on the left side of the screen.
6. On the right side of the screen, select “Health Concerns” from the menu on the top of the screen.
7. Under “Health Concerns,” select “Report a Health Concern” from the menu on the left side of the screen.
Once you have logged in, please review their health concern policy and then select “Submit Health Concern” from the menu on the right side of the screen.
How to cancel your account
