In this tutorial, we will show you how to login to the Ssm Health Portal using your username and password.
How to login to Ssm Health Portal
If you are new to Ssm Health Portal, or if you have forgotten your password, please follow these steps to login:
1. Click the login link in the top right corner of any page on the portal.
2. Enter your username and password in the fields provided and click submit.
3. You will be redirected to the login page for your account type (Community Member, Patient, or Caregiver).
4. If you are a Community Member, Patient, or Caregiver, click the link in the email that was sent to you after you registered for an account on the portal. This link will take you to your account login page.
5. If you are a Member of a patient's support group, or a Caregiver for a patient with dementia or another chronic condition, please contact your group's coordinator to get your login information.
How to find your account details
If you have forgotten your password, or if you have lost your login details, you can find them on the Ssm Health Portal.
To log in to the Ssm Health Portal:
1. Go to the Ssm Health Portal at www.ssmhealthportal.com
2. In the top left-hand corner of the website, click on “My Account”
3. On the “My Account” page, under “Account Details”, enter your username and password (or login details)
4. If you have forgotten your password, you can also click on “Forgot Password?” and enter your email address and new password in the provided fields
5. Click “Submit” to log in
How to change your password
If you have forgotten your password, please follow these steps to change it:
1. Log in to the Ssm Health Portal.
2. On the top right corner of the screen, select “My Profile”.
3. In the “My Profile” section, click on the “Password” link.
4. Enter your current password in the appropriate field and click on the “Change Password” button.
5. You will be asked to confirm your new password. Click on the “Confirm New Password” button and you are done!
How to report a problem with Ssm Health Portal
If you experience any difficulties logging into Ssm Health Portal, please report the issue using the “Contact Us” form on the website. We would appreciate it if you could include the following information in your email:
- Your name and contact information
- The web browser and operating system you are using
- The problem you are experiencing
- The date and time of the problem
How to unsubscribe from their email updates
To unsubscribe from their email updates, follow these instructions:
1. Go to the “My Account” page on their website.
2. Under “Email Updates,” select the checkbox next to “I want email updates about Ssm Health content only.”
3. Click the “Unsubscribe” button.
