2. You will be taken to a login screen. Enter your name (first and last) and email address, then click on “submit”.
3. You will now be taken to the main Patient Portal page. On the left-hand side of the page is a list of sections; select “My Medical History”. This will take you to a page where you can enter all of your medical information.

Viewing your medical records
If you are a patient at Southampton Healthcare, you can use their patient portal to view your medical records. Managing your account
