How to login to the South Lake Hospital Patient Portal
If you have not already done so, please create a user account on the South Lake Hospital Patient Portal. You will need your login name and password to access the portal. To create your account, click the link below. After you have registered, you will be able to log in by clicking on the “Login” tab at the top of the homepage.

How to find your doctor and schedule appointments
South Lake Hospital is committed to providing timely, quality health care. To find your doctor or schedule appointments, navigate to their Patient Portal. The Patient Portal is available 24 hours a day, 7 days a week.
To login to the Patient Portal:
1. Navigate to www.southlakehospital.com and sign in.
2. Click on the “Patient Portal” link at the top of the home page.
3. Enter your Patient ID number located on your hospital ID card or driver’s license into the login form and click on “Log In.”
4. Enter your name and password into the login form and click on “Log In.”
5. The Patient Portal will open in a new window displaying all of your current records and appointment information.
How the Patient Portal works
If you are a patient at South Lake Hospital and have an account with them, you can use their Patient Portal to access your medical records, view your appointment calendar, and more! To login to the Patient Portal, follow these steps:
1. Log in to your MySouthLake account. If you don't have an account, sign up now.
2. Click the Patient Portal link in the navigation bar on the left side of the MySouthLake website.
3. Enter your MySouthLake username and password in the appropriate fields and click Log In. You'll be prompted to select a hospital department from which to view your records.
4. To access other features of the Patient Portal, click on one of the tabs at the top of the screen: Medical Records, Appointments, or Messages.

If you have a problem logging in or finding your health information, please follow these steps:
1. Verify that your email address is correct.
2. Confirm that you are using the correct password.
3. Make sure your computer is connected to the internet and that the browser is up-to-date.
4. Try entering your login information again (for example, your last name and first initial).

If you are not already registered with the South Lake Hospital Patient Portal, please follow these steps to create an account:
1. Go to www.slhospitals.com/patientportal and click on "Sign In" in the top right corner of the homepage.
2. Enter your name and email address in the appropriate fields, and click on "Sign Up."
3. You will be asked to create a password. Password must be at least 8 characters long and include at least one letter and one number. Click on "Create Password" to create your password.
4. Once you have logged in, you will see the main patient portal page. To access different sections of the portal, click on the tabs in the top left corner of the page: "My Account," "Health & Wellness," "Services," and "News."
