If you are a family physician and have registered with the Sound Family Medicine Portal, you can access your health history. You will need to enter your registration number and password. Once you have logged in, click on My Profile on the left-hand menu. Under Health History, you will see a list of all of your health records. You can view dates of health events, medications taken, and other information.
How to manage medications
Sound Family Medicine Portal allows registered users to manage their medications, including adding, editing and deleting prescriptions. To login, click the "Log In" link in the top right corner of the screen. Enter your user name and password, and click "Log In." You will then be taken to the "Medications" screen. Under "Patient Profile," click on the "Medications" tab. On this screen, you will see a list of all of your medications. To add a new medication, click on the "Add New" button and fill out the required information. To edit an existing medication, select it and click on the "Edit" button.
