How to login to Select Health Provider Portal
To login to the Select Health Provider Portal, follow these steps:
1. Log in to your personal account on the Select Health Provider Portal. If you have not already registered for an account, you can do so by clicking on the "Register" tab at the top of the portal and following the instructions.
2. Click on "Login."
3. Enter your user name and password. If you have forgotten your user name or password, click on the "Forgot Your Password?" link at the bottom of the login screen and follow the instructions.
4. Click on "Log In."
How to change your password
If you have forgotten your password, you can change it by following these steps:
1. Log in to the Select Health Provider Portal.
2. Click on the "Password" link in the header bar.

1. Navigate to the Select Health Provider Portal home page and click on the "Add a New Health Provider" link in the upper-right corner of the page.
2. On the Add a New Health Provider page, enter the desired information about your new health provider, including their name, contact information, and website address.
3. Click on the "Save" button to finalize your entry.
How to remove a health provider from the Select Health Provider Portal
If you no longer need access to a health provider on the Select Health Provider Portal, you can remove that provider from the portal by following these steps:
1. Log into the Select Health Provider Portal at www.selecthealth.gov.
2. Click on the "Provider" link on the main menu bar.
3. Find the health provider you want to remove and click on their name in the list of providers.
4. On the "Provider Profile" page that appears, click on the "Remove this Provider" link next to the health provider's name.
Please note that removing a health provider from the Select Health Provider Portal will remove that provider from all of your personal health records with Select Health and could impact your ability to receive care from Select Health providers in the future.
