If you are a Scottsboro Family Physicians patient, you can use the Patient Portal to access your medical history. To find your medical history on the Patient Portal, follow these steps:
1. Click on the "My Medical History" link on the home page of the Patient Portal.
2. On the My Medical History page, click on the "View My Medical History by Provider" link under the "Scottsboro Family Physicians" header.
3. On the View My Medical History by Provider page, select the "Scottsboro Family Physicians" provider from the drop-down menu and click on the "View My Medical History" button.
4. On the View My Medical History by Provider page, you will be able to view your medical history by date and provider. You will also be able to create a health chart and add notes about your visits to Scottsboro Family Physicians.

2. Once you have logged in, select “My Doctor” from the menu on the left-hand side of the screen. This page will list all of your doctors who are registered with Scottsboro Family Physicians. Each doctor has a “Profile” page that includes information such as their name, specialty and website address. You can click on a doctor’s Profile to view their complete medical record and contact information.
