If you have forgotten your username or password, click on “forgot my username or password?” on the top right-hand corner of the login screen and enter your email address and the last four digits of your social security number. A reset code will be sent to this email address. Once you have received this code, enter it into the login screen and click “ reset password.” You will then be able to log in using your new username and password.
3. If you are a registered patient, please enter your patient number in the login screen and click “register now.” Your patient number can be found on your medical record or on their website under My Account/Login Details (look for Patient ID).
