How to login to the Provida Patient Portal
If you have not already done so, please login to the Provida Patient Portal by following these simple steps:
1. Go to www.providahealthsystem.org and click on the “Patient Portal” link in the top left corner of the homepage.
2. Enter your patient number (or unique identification number if you are a new patient) and password in the respective fields and click on “Log In”.
3. You will be taken to a page where you can view your account information, including your medical records, appointment history, and more!

How to find your medical records
If you are a Provida Family Medicine patient, you can use the Patient Portal to find your medical records. To login to the Patient Portal, first go to their website at www.providafamilymedicine.org and click on the Patient Portal link in the navigation bar on the left side of the screen. Once you are on the Patient Portal, enter your email address and password in the appropriate fields and click on the Login button. You will now be able to access all of your medical records from their database. The Provida Family Medicine Patient Portal is a secure online resource that allows you to connect with your Provida family medicine doctor and access health information, medication reviews and other resources. To login, follow these steps:
1. Go to www.providafamilymedicine.com and enter your patient ID or unique referral number (if you are a new patient). You will be prompted to create a password.
2. Once you have logged in, click on My Account at the top of the page to view your account information. You can also access recent health history and medication logs from this screen.
3. Click on My Profile to view important personal information about yourself, such as your name, address and phone number. This information will be shared with your Provida family medicine doctor if you have registered with this service.
