How to login to the Proact Patient Portal
The Proact Patient Portal is a secure online portal that allows patients to view their medication history, refill and pharmacy order information, and communicate with their healthcare provider. To access the Patient Portal, patients need to login using their patient ID and password.
To create a new account, click the "Create an Account" link on the main menu bar of the Patient Portal. Once you have created your account, you can log in by clicking the "Login" link on the main menu bar of the Patient Portal and entering your patient ID and password.
How to manage your account
If you are a registered patient of Proact Pharmacy and have an account, login here. You will be able to manage your account, track your prescription history, and more!
How to update your contact information
If you have registered with their patient portal, you can update your contact information by logging into the portal and clicking on the "My Profile" link in the top navigation bar. This will take you to a page where you can enter your new contact information.
How to unsubscribe from email notifications
To unsubscribe from email notifications, please follow these steps:
1. Log in to your Proact pharmacy patient portal account.
2. Click on the notification icon located in the top right corner of the homepage.
3. Select "Unsubscribe" from the drop-down menu next to your email address.
4. Click on "Submit" to unsubscribe from all email notifications.
How to report a problem
If you have a problem with the Proact pharmacy patient portal, you can report it on their website.:
1. Log into the Proact pharmacy patient portal.
2. Click on the “Report a problem” link in the top right corner of the screen.
3. Fill out the form and provide as much detail as possible about your issue.
4. Click submit to send your report to us.
How to dispute a charge

3. Once you have logged in, use the menus at the top of the screen to explore all of the features of their Patient Portal.
