If you have a family care patient portal account, you can log into the system to view your medical records, make changes, and more. Follow these steps to access your file:
1. If you don't have an account yet, create one now.
2. Go to the patient portal home page and click on the "Login" link in the upper-left corner.
3. Enter your user name and password in the appropriate fields and click on the "Log In" button.
4. You will be taken to the main patient portal screen. On the left side of the screen, you will see a list of menus. Click on "My File." This will take you to a file overview screen where you can view your records by date, type of record (medical history, medication logs, etc.), or category (family care activities such as wound care).
If you would like to make changes to your file (such as adding a new doctor), click on "My File" again and choose from among the available menu options.

If you are a family care patient and would like to login to their patient portal, please follow these steps:
1. Log in to your healthcare providerβs website.
2. Click on the βMy Pageβ link near the top of the page.
3. Click on the βPatient Portalβ link in the menu on the left side of your screen.
4. Enter your Patient ID number and password in the appropriate fields to log in.

How to Cancel Your Account
The Patient Portal is a great way to keep track of your health and progress. However, if you need to cancel your account, there are a few steps you need to take.
First, log in to the Patient Portal.
Next, click on the Account link in the main menu.
On the Account page, find the Cancel My Account link and click on it.
You will be asked to confirm your cancellation. Once you have confirmed your cancellation, your account will be cancelled and you will no longer be able to access or use the Patient Portal.
How to Change Your Password
If you have forgotten your password, please follow these steps to reset it:
1. Click on the "My Account" tab at the top of the portal.
