How to login to the Physicians Healthcare Network Patient Portal
If you are a physician or patient, you can use their Patient Portal to access your medical records and communicate with your healthcare team. Logging in is easy:
1. Click the "Log In" button on the front page of the Patient Portal.
2. Enter your username and password.
3. Click "Log In."
How to find your account number and password
If you forgot your password or account number, please follow these steps:
-Click on the "Forgot your password?" link in the login screen.
-Enter your email address and click on the "reset password" button.
-Your new password will be sent to your email address.
-Click on the "Login" button to sign in to your account.
How to update your personal information
If you have not done so already, please update your contact information by clicking on the "Update Personal Information" link at the top of the Patients Healthcare Network Patient Portal. Once you have updated your contact information, please click on the "Login" link to begin the login process.
How to cancel your account
If you decide that you no longer want to use the Physicians Healthcare Network Patient Portal, there are several steps you need to take in order to cancel your account. The first step is to login to your account and find the "Cancel Account" link on the main menu. After clicking on the "Cancel Account" link, you will be taken to a page where you can enter your account password and click on the "Confirm Password" button. After clicking on the "Confirm Password" button, you will be taken to a page where you can enter your email address and click on the "Submit" button. You will then be taken to a confirmation page where you will need to click on the "Update Profile" button in order to finalize your cancellation.

If you have a dispute with your bill, first check to see if the bill is correct. If it is not, you can dispute it. Dispute forms are available online at www.pchwnetwork.org and from many physician offices. You may also call the billing department at 1-800-MEDICARE (1-800-633-4227).
Once you've disputed the bill, the billing department will investigate the dispute and send you a final bill or adjustment notice. If you still disagree with the final amount, you can ask for a hearing before an independent review organization.
