Physicians Health Network Provider Portal is a web-based health care management system that allows physicians to electronically submit and manage patient information. In order to login to the Portal, you will need your patient's name and provider number.
How to login to the PHN Provider Portal
If you are a PHN provider, you can login to their Provider Portal to view your account information, update your contact information, and more. To login, visit their website and click the "Provider Portal" link in the navigation bar at the top of the page. You will be prompted to enter your login credentials. If you have not registered for the Provider Portal yet, you will be asked to do so before continuing. Once logged in, you will see a list of all of your accounts on the Provider Portal. You can also view detailed account information by clicking on an account name on the main Provider Portal page.
How to find a provider
If you're looking for a physician, the Physicians Health Network Provider Portal is a great resource. The portal includes information on all PHN provider locations and services. You can search by specialty or location.
To login, first create an account. If you don't have an account yet, you'll need to create one before you can login. Once you have an account, click the Login link at the top of the page. Enter your username and password and hit Log In.
On the Provider Portal home page, click the My Profile link in the top right corner of the screen. This will take you to a screen where you can enter your name, email address, and other contact information. You'll also need to provide your billing ZIP Code so that we can send you important notices about your account and health care services.
Next, under My Profile, click My Services to view a list of all of your active PHN provider services. You can see what services are included with your membership plan and which ones are optional. Scroll down until you find the service that you're interested in and click it to view more detailed information about that service.

How to search for a provider by specialty
If you are looking for a specialist in a specific field, the Physicians Health Network Provider Portal can help. You can use the portal's search function to find providers by specialty.
To search for providers by specialty, first login to the portal. Once you are logged in, click on the "Specialty Search" link in the navigation bar at the top of the page.
In the "Specialty Search" window, enter your medical specialty in the "Search By" box and then click on the "Search" button.
The Physicians Health Network will display a list of providers who meet your criteria. To view more information about a provider, such as their contact information, click on the provider's name or image.
How to view your account information
If you have not already registered with PHN, please do so now. Once you have registered and logged in, use the links below to view your account information. If you have any questions about how to login or view your account information, please contact their customer service team at 1-800-477-2327.
To view your account information:
1) Click on the "Account" tab on the main PHN website (www.physicianshealthnetwork.com).
2) In the "Personal Profile" section, click on "My Account."
3) On the next page, under the "My Account Status" heading, you will see your current registration status and your login credentials. If you have forgotten your login information, please click on the link below to reset your password. After resetting your password, please enter it in the appropriate box on the next page and click on the "Reset Password" button.
4) After logging in, you will be able to review your account activity and health insights data. You can also manage your personal settings and preferences by clicking on "Manage My Profile."

How to report a concern
Physicians Health Network Provider Portal
If you have a concern about a PHN provider, you can use the online provider portal to report the concern. The provider portal provides easy access to your account, contact information for your PHN provider, and other important information.
To login to the provider portal, visit: www.physicianshealthnetwork.org/ProviderPortal.aspx. Once you are logged in, click on "My Account" in the top left corner of the screen. On the My Account page, under "Contact Information," click on "Report a Concern." You will be taken to the Report a Concern form. In the "Details" section, please provide as much information as possible about your concern. For example, if you are concerned about a PHN provider's care of a patient, include the patient's name and address. If you are concerned about a PHN provider's billing practices, include specific examples of what you believe is wrong with the billing. You can also attach documents if you have them. Click on "Submit" to submit your concern.
How to unsubscribe from email notifications
If you no longer wish to receive email notifications from the Physicians Health Network Provider Portal, you can unsubscribe by following these steps:
1. Log in to the Provider Portal.
2. Click on the "My Account" tab.
3. Under "Email Notifications" on the left-hand side, click on the "Unsubscribe" link next to the email notification you wish to unsubscribe from.
