What is a Patient Portal?
Patient Portal is a secure website that patients can use to access their health information. Patients can use the Patient Portal to access their health data, order prescriptions, find health care resources and more.
How to Login to Your Patient Portal?
To login to your Patient Portal, follow these steps:
1. Go to the patient portal website (https://patientportal.accenturehealth.com).
2. Click on the “Log In” button in the top-right corner of the homepage.
3. Enter your user name and password in the appropriate fields, and click on the “Log In” button.
How do I sign up for a Patient Portal?
If you are a patient and would like to sign up for a Patient Portal, please follow these steps:
1. Go to the main menu on the website and select “Patient Portal”.
2. Click on the “Register for Patient Portal” link in the upper right corner of the page.
3. Fill out the required information and click on the “Register for Patient Portal” button. You will be taken to a confirmation page where you will need to enter your email address and password.
If you have forgotten your password, please click on the “Forgot Your Password?” link on the confirmation page and create a new password. Once you have entered your new password, you will be able to log in to your Patient Portal account.
How do I login to my Patient Portal?
If you are a new Patient Portal user, please follow these steps to login:
1. Navigate to the home page of your Patient Portal. You can find this page by clicking on "My Account" in the upper-right corner of your screen or by searching for "Patient Portal" on the homepage.
2. Click on the "Login" button in the upper-left corner of the screen.
3. Enter your username and password in the appropriate fields and click on the "Login" button. You will be logged in to your Patient Portal!
If you have already logged in to your Patient Portal, please follow these steps to login:
1. Click on the "Login" button in the upper-left corner of the screen.
2. If you are currently logged in, you will be prompted to enter your username and password. If you are not currently logged in, you will be prompted to create a new account or log in to an existing account.
3. Enter your username and password in the appropriate fields and click on the "Login" button. You will be logged into your Patient Portal!
Where can I find my Patient Portal ID and Password?
If you did not receive a Patient Portal ID and Password when you registered for your account, please follow these steps to login:
1. Log in to your account by clicking the "Login" button on the homepage.
2. Click the "Account Settings" link in the left-hand menu.
3. Enter your Patient Portal ID and Password in the appropriate fields, and click the "Update" button.
