As patients, it's their right to have access to all of their medical information. This includes their medical history, medications, and any other relevant information. But what if you can't remember your username or password? How are you supposed to manage your health information if you can't even log in?
How to create an account
If you don't have an account, create one now.
To create an account, first visit the patient portal home page.
Click the "Sign In" link in the top right corner of the page.
Enter your user name and password.
You'll be taken to a login screen.
Type in your email address and password, and click "Login."
If you're already signed in, you can skip to Step 5.

2. Enter your username and password.
3. Click Sign in.

6. Under "Sign In Method," select Google sign-in (or whatever sign-in method you're using for your other online accounts).
7. Under "Confirm Sign In," enter your email address and click Confirm Sign In.
8. Your patient portal will now be ready for use!
Login with Email
If you have an email address registered with your health care provider, you can login to your patient portal using that email address.
To login using your email address:
1. Open your patient portal.
2. Click "Login." (You may need to sign in first.)
3. Enter your email address in the "Email" field and click "Submit."
4. You will be redirected to the message confirming your login. If you are not redirected, please check that your email is correct and try again. If you still cannot login, please contact customer service for assistance.
