Registering with the portal will allow you to easily keep track of your appointments, receive reminders about upcoming tests and treatments, and more. Here is how you can login to the patient portal.
How to login to Patient Portal
If you are a registered patient or caregiver, you will need to login to the Patient Portal to view your medical records and make changes. To login, follow these steps:
1. Go to www.patientsafety.gov and sign in with your credentials.
2. Click on the “Patient Portal” link in the left navigation bar.
3. Enter your patient ID number (found on your hospital or clinic billing statement) into the “Patient ID” field and click on the “Login” button.
4. Enter your password into the “Password” field and click on the “Login” button.
5. If you have created a My Account account, you will be prompted to enter your My Account username and password in the appropriate fields after clicking on the “Login” button. If you have not created an account, simply enter your patient ID number and password into the appropriate fields and click on the “Log In” button.
Once you have logged in, you will be taken to the main Patient Portal page where you can view your medical records,
Finding your login information
If you are a new patient or have not logged into their patient portal in the past, please follow these easy steps to find your login information:
1. Click on the "Login" link located on the top navigation bar of the home page.
2. On the login screen, enter your email address and password. If you have not created a password, please click on the "Create Password" link and enter your desired password.
3. Click on the "Login" button to log in to their patient portal.
