How to request a leave of absence
If you are an employee at Parkview Hospital and need to take a leave of absence, the process is as follows:
1. Login to your Employee Portal account.
2. Click on the My Account tab.
3. On the left hand side, under Leave of Absence, select Request a Leave of Absence.
4. In the Request for Leave of Absence form, please provide your name, position, start and end dates of leave, reason for leave and contact information. You will also be asked to sign a release form authorizing your supervisor to release your records should you return to work after your leave. Submit this form to your supervisor.
5. Your supervisor will review your request and either approve or deny it based on the availability of resources and the needs of the hospital. You will be notified in writing of the decision made regarding your leave of absence.
How to submit a grievance
If you have a problem with your job or with the way your employer is treating you, there are a few things you can do to try to get resolution. The most common first step is to speak to your supervisor or HR representative, but if that doesn't work, you may be able to take steps through the grievance process.
