To find your account number, login to their patient portal and click the “My Account” tab. Enter your first and last name in the fields provided and hit the “Search” button. Your account number will appear in the results.
How to change your password
If you've forgotten your password, or if you would like to change it, follow these steps:
1. Click on the "My Account" link on the homepage of their website.
2. In the "My Account" page, click on the "Forgot Password" link.
3. Enter your email address in the "Email Address" field and click on the "Create Password" button.
4. Confirm your new password by clicking on the "Confirm New Password" button.
5. Click on the "Log In" link at the bottom of the page to log in to your account.
