How to Log In
If you are a new patient, you will need to create an account on the Orlando Family Physicians Patient Portal. To create an account, click on the "Sign In" button located in the top right corner of any page on the portal. You will be prompted to enter your name, email address, and password. Once you have logged in, you will be able to access all of your personal information and medical records.

How to See Your Health History
If you're a patient of an Orlando family physician, you may be wondering how to access your health history. The Patient Portal is a great way to get started! Here's how to login:
1. Go to the Patient Portal website at www.orlandofamfam.com/patient-portal.
2. Click on "Login" in the upper right corner of the screen.
3. Enter your name and email address in the appropriate fields, and click "Sign In."
4. You'll be asked to confirm your password. Click "Confirm Your Password."
5. You'll now be taken to your account overview page. On this page, you'll see all of the medical records associated with your account. To see your health history, click on "Health History."
How to Request a Medical Record
If you are a patient of one of their Orlando family physicians, and would like your medical record sent to you electronically, please follow these simple steps:
1. Go to their website and click on "My Account" in the top right corner.
2. Click on the "Documents Requested" tab.
3. Click on "Medical Record Request."
4. Fill out the form as best you can and provide as much information as possible. Please include your full name, date of birth, address, phone number and email address. You may also include any additional health information that you feel is important to share with them. They will contact you to verify the information you have provided and to determine if a medical record is available for download. If we are unable to provide a medical record for download, we will provide instructions on how to request a copy by mail. Thank you for your interest in their practice!
