How to login to the Patient Portal
If you are a patient at Ocala Family Medical Center, you can use their Patient Portal to access your health information, pay your bill, and more! Here are the steps to login:
1. Click the "Patient Portal" link on the main navigation bar.
2. In the Patient Portal window, enter your Patient ID and Password.
3. Click the "Login" button to log in.
4. Review your account information and make any changes if necessary.
5. To access your health information, click the "My Health Record" link on the left side of the Patient Portal window.
6. Enter your name and date of birth to begin accessing your health records. You can also view discharge summaries and event logs from previous visits to their hospital.
How to find your account number and password
If you have forgotten your account number or password, please follow these steps:
1. Log in to the Patient Portal.
2. Click on “My Account” in the header bar.
3. Enter your patient number and password in the appropriate fields.
4. Click on “Forgot Your Password?” below your account information to reset your password.
How to update your personal information
If you have ever changed your name or address, or if you have created a new account, please follow these steps to update your personal information:
1. Log in to the Patient Portal.
2. Click on "My Account" in the menu bar at the top of the page.
3. In the "My Account" section, click on "Edit Personal Information."
4. Enter your current name and address in the appropriate fields and click on "Submit."
5. You will now receive an email notification confirming that your information has been updated. Be sure to check your email inbox for this message!
How to cancel your account
If you need to cancel your account with the Ocala Family Medical Center Patient Portal, follow these steps:
1. Log in to the Patient Portal.
2. Click on My Account near the top of the page.
3. Under Services, click on Cancel My Account.
4. Follow the prompts to cancel your account.

If you are not already registered with the Patient Portal, please follow these instructions to register and create a new account. Once you have registered and logged in, you can access all of the resources available on the Patient Portal.
Once you have logged in, click on "My Account" in the top right corner of the screen. In the My Account screen, under "Log In," enter your email address and password. Click on "Log In" to complete your registration process.
