How to update your contact information
If you would like to update your contact information on the North Coast Family Medical Group Portal, please follow these simple steps:

If you have a health concern and would like to report it through the North Coast Family Medical Group Portal, here are the steps you need to follow:
1. Log in to the North Coast Family Medical Group Portal.
2. Click on "My Account" in the top right corner of the screen.
3. Click on "Health Concerns" in the left navbar.
4. In the "Health Concerns" section, click on "Report a Health Concern."
