How to Request a Copy of Your Records
If you would like a copy of your records from their clinic, please visit their Patient Portal and login. From there, you can select Request a Copy of Your Records. You will need to provide your name, date of birth, and clinic identification number. They will then send you a copy of your records.

If you have a broken screen on your Westmed Patient Portal, here is how to login and report the issue.
To login to your Westmed Patient Portal, first click on the “Login” button in the upper right corner of the homepage.
You will be asked to enter your password. After you have logged in, you will be taken to the “My Account” page.
On the “My Account” page, you will see a list of all of your devices that are connected to your Westmed Patient Portal account. You can click on the “Broken Screen” button next to the device that has a broken screen.
