Hazen is a patient portal that allows patients to manage their healthcare information securely online. As a registered user, you can create an online profile, schedule appointments, and view your medical records. To login to your Hazen Patient Portal, follow these steps:
1) Log in to your My Hazen account.
2) Select "Patient Portal" from the menu on the left.
3) Enter your email address and password in the appropriate fields.
4) Click "Login." You will be prompted to confirm your password.
How to create an account
If you are a new patient, creating an account is easy. Simply click on the "Create an Account" link on the main page, and follow the instructions. If you already have an account, you can login by clicking on the "Login" link in the upper right corner of the page.
How to login
If you're a new patient, or have forgotten your username and password, follow these instructions.
1. Log in to the My Hazen Patient Portal using your email address and password. If you've forgotten your username and password, click the "Forgot Your Username or Password?" link on the left side of the homepage. You will be prompted to enter your email address and password. You will then be redirected to the login page.
2. Enter your email address in the "From" field and your desired username in the "To" field. Click "Send Email." The system will send you an automated welcome message with instructions for logging in. If you do not receive this message, please check your spam folder.
3. Click on the link in the automated welcome message to log in to the My Hazen Patient Portal. You will be directed to a page where you can select which services you would like to view or register for.
How to view your medical records
If you are a MyHazen patient, you can view your medical records on their Patient Portal. To login to the Patient Portal, follow these steps:
1. Log in with your MyHazen account username and password.
2. Click on the "My Medical Record" tab.
3. You will be prompted to enter your doctor's name and email address. If you have not already registered with MyHazen, please do so now by clicking on the "Register" link below the doctor's name and email address fields. Once you have registered, you will be able to login using your registered information.
4. Click on the "View my medical record" link under each of your doctors' profiles to view your medical records.
How to request a copy of your medical record
If you would like to request a copy of your medical record, please follow these steps:
Step 1: Log into your Hazen Patient Portal.
Step 2: Click on the "My Health History" tab.
Step 3: Under "My Records Requested," select the "Request Copy of Medical Record" option.
Step 4: Complete the requested information and click submit. A copy of your medical record will be emailed to you within 24 hours.

2. Enter your username and password, and click "sign in."

If you are looking for a clinic near you, the My Hazen Patient Portal can help. The portal is a one-stop shop for information on clinics and healthcare providers in your area.
To find a clinic near you, click on the "Clinics" tab on the My Hazen Patient Portal home page. This will list all of the clinics that are affiliated with My Hazen. You can then filter the list by location, type of care, or specialty.
If you don't see a clinic that meets your needs right away, use the "Contact Us" tab to send us your information and we'll do their best to connect you with a clinic near you.
How to make a payment
When you are ready to make a payment, follow these steps:
1. Log in to your Hazen Patient Portal account.
2. Click on the "Payments" tab.
3. Click on the "Make a Payment" button.
4. Enter the amount you would like to pay and select the payment type (cash, check, or credit/debit card).
5. Review the information and click on the "Submit Payment" button.
