How to Login to the Msmg Patient Portal
The Msmg Patient Portal is a secure website that can be used to manage your health information. To login, please follow these steps:
1. Click the "Login" link on the top of the page.
2. Enter your username and password in the appropriate fields.
3. Click "Log In." You will now be able to access all of the features of the Msmg Patient Portal.
How to Update your Personal Information
If you have recently changed your name or address, or if you would like to update your contact information, please follow these steps:
1. Go to the MySMG Patient Portal.
2. In the top left corner of the screen, click on My Profile.
3. Click on Update Personal Info.
4. Complete the form and click on Submit.
How to Request a Copy of Your Medical Records
If you would like a copy of your medical records, you can login to the Mitsubishi Motors Manufacturing Group Patient Portal and request a copy.
To login to the Mitsubishi Motors Manufacturing Group Patient Portal, visit www.mmmgp.com and sign in using your username and password. Then click on the “My Account” link in the top left corner of the main page. On the “My Account” page, click on the “My Health Records” link in the left column.
Click on the “Request A Copy Of Your Medical Records” link under My Health Records. You will be asked to provide your name, date of birth, contact information (including email address), and reason for requesting a copy of your medical records.
You will then be directed to a confirmation page where you will be able to confirm that you want a copy of your medical records and provide additional information if necessary. Once your request has been confirmed, a copy of your medical records will be sent to the email address that you provided.
How to Report a Complaint or Concern
If you have a complaint or concern about your health care experience, you can use the patient portal to communicate your concerns. The patient portal is an online tool that allows patients to access their health information, including charts and medications. To log in to the patient portal, follow these steps:
1. Go to www.msmg.gov/patientportal and sign in with your MSMG ID and password.
2. On the home page, click on My Health Stories.
3. Click on the link for Reporting a Concern or Complaint under My Health Stories.
4. In the Reporting a Concern or Complaint form, please let us know what type of concern or complaint you have (for example: medication error, communication issue). Please also include as much detail as possible (for example: when did the issue occur, what happened, who was involved). If you are reporting a serious issue (for example: a death), please email us at [email protected] instead of filling out the form online.
