Montgomery Cardiology Associates Patient Portal is a secure website that allows patients to access their medical records, request appointments and view their health history. To login, patients must have a valid email address and password. The patient portal is available 24 hours a day, seven days a week.
How to find your account number
Montgomery Cardiology Associates Patient Portal account numbers are located on the back of your ID card. To find your account number, log in to the patient portal and go to "My Account." Next to "View My Profile" is a picture of your ID card. The account number will be listed in the lower right-hand corner.
How to find your patient portal password
If you forgot your password, or if you have not used your patient portal in a while and would like to log in, follow these simple steps:
1. Log in to the patient portal using your email address and password.
2. Click on "Forgot Your Password?"
3. Enter your email address and click on the "reset password" button.
4. You will receive an email with a new password. Once you have confirmed that you have received the new password, enter it into the login form on the main page of the portal and you are ready to go!
How to change your password
If you have forgotten your password, first try logging in using the email address associated with your account. If that doesn't work, please contact them at [email protected] and we will help you reset your password.

How to activate your account
If you are a new patient, or have not had a chance to activate your account yet, please follow these simple steps:
1. Log into the website at www.montgomerycardiologyassociates.com.
2. Click on the βMy Profileβ link on the top right corner of the homepage.
3. On the My Profile page, click on the βActivate My Accountβ link in the left sidebar.
4. Enter your email address and password in the appropriate fields, and click on the βActivate My Accountβ button to finish activating your account.
If you have logged into your account before, simply enter your username and password in the appropriate fields and click on the βLog Inβ button to continue.

How to update your contact information
If you have changed your contact information in the Patient Portal, please follow these steps:
1. Log into the Patient Portal.
2. Click on "My Account" on the left-hand side of the screen.
3. In the "My Profile" section, click on "Contact Info."
4. On the "Contact Info" page, under "Personal Info," click on the blue link that says "Update Contact Info."
5. Fill out the fields with your new contact information and click on "Update Contact Info."
How to unsubscribe from receiving email messages
To unsubscribe from receiving email messages from Montgomery Cardiology Associates, please follow these instructions:
1. Log into the patient portal.
2. Click on the "Email Preferences" link located in the header of the homepage.
3. Uncheck the box next to "I want to receive email updates from Montgomery Cardiology Associates."
4. Click on the "Save Preferences" button at the bottom of the page.
