How to login to the Mission Primary Patient Portal
To login to the Mission Primary Patient Portal, follow these steps:
Step 1: Go to missionprimary.org and click on the “Login” button in the top right corner of the homepage.
Step 2: Enter your user name (first name initial) and password. If you have forgotten your user name or password, please contact them at [email protected].
Step 3: Click on “Log In” to complete your login process.
How to find your medical history on the Patient Portal
If you have ever had a medical procedure or visited the doctor, chances are good that your medical records are stored in one or more electronic health record (EHR) systems. Mission PrimaryCare's Patient Portal is one such EHR and allows patients to access their medical history and other health information. Here is how to find your medical history on the Patient Portal:
1. Log in to the Patient Portal using your account name and password. If you don't have an account, click "Create an Account" at the top right of the page.
2. Click on "My Medical History." This will take you to a page where you can view all of your appointments, test results, treatments, etc.
3. If you would like to see your personal health summary, click "View My Health Summary." This will show you a list of all of your conditions and medications, as well as how well they are working.
How to update your personal information on the Patient Portal
To update personal information on the Patient Portal, log in to your account and follow these steps:
1. Click on the "My Account" tab on the top left of the page.
2. Click on the blue "Update Personal Information" button.
3. Enter your name, email address and password in the corresponding fields and click on the "Update Personal Information" button to complete the process.
How to request a copy of your medical records from Mission Health
If you are a Mission Health patient and would like to request a copy of your medical records, please follow these steps:
Login to the Mission Primary Patient Portal. Select "Request Records" from the menu on the left. Complete the form including your name, date of birth, and contact information. Click "submit". Your request will be reviewed and processed within two business days. A confirmation email will be sent to you with your records attached. How to report a safety concern about Mission Health
If you have a safety concern about Mission Health, please use their patient portal to report the issue. The patient portal is a secure online system that allows patients and families to communicate with Mission Health staff about their care. You can use the patient portal to report any concerns you have about your health or the care you received from Mission Health.
To access the patient portal, please click here. Once you are logged in, please follow these steps to report a safety concern:
1. Click on the "My Concerns" tab on the top of the page.
2. In the "Description of My Concern" box, type in as much information as you can about your concern. For example, what happened, who was involved, where it occurred, etc.
3. In the "Additional Information" box, provide any additional information that would help us understand your concern. For example, how long has this been going on, has it happened before, etc.?
4. Click on the "Submit My Concern" button at the bottom of the page.
5. You will be taken to a confirmation page where you can review your submission and submit another concern if needed.
