Mid Cities Family Care is a patient portal that provides access to medical records, medication lists, and more for patients living in the Mid Cities area. The patient portal also offers a variety of other resources such as nutrition information, health education materials, and community events. Patients can sign in to their account using their patient ID number or their name and password.
How to Login:
To sign in to your Mid Cities Family Care account, you will need to have your patient ID number or your name and password. To get your patient ID number, you can visit their website or call us at (713) 527-5200. To get your name and password, you can email us at [email protected] or call us at (713) 527-5200.
How to Login to the Patient Portal
To access the Patient Portal, patients must first login. To do so, patients can visit the "login" link on the main navigation bar of the website. After logging in, patients will be able to view their account information, medication history, and more.

How to change your password
If you have forgotten your password, or would like to change it, follow these steps:
1. Log in to the portal at www.midcitiesfamilycare.com.
2. Click on the "My Account" link in the top left corner of the screen.
3. On the My Account page, click on the "Password" link in the login column next to your name.
4. Enter your current password in the "Password" field and click on the "Change Password" button.
5. Your new password will be displayed in the "New Password" field and you can click on the "Apply New Password" button to finish setting it up.
Lost your Patient Portal password?
If you have forgotten your password, please follow these steps to reset it:
1) Log in to your Patient Portal account by clicking on the "Log In" link in the top right corner of the homepage.
2) If you have not previously entered your Patient Portal password, it will be pre-populated for you. To change or reset your password, click on the "Password Change" link in the login form.
3) Enter your new password twice in the required fields and click "Submit."
4) If you still cannot log in after following these steps, please email [email protected] with your name and Patient Portal username, as well as a screenshot of your login screen. They will help you reset your password as soon as possible.
