How to Login to Meritus Patient Portal
If you are new to Meritus Patient Portal, or need to login for the first time, follow these simple instructions.
1. Log in to Meritus Patient Portal using your email address and password (if you have registered for an account). If you have forgotten your password, click forgot my password link at the top of the page.
2. Click on the Patients tab to view all of your patient records. You can search for a patient by name, admission date or condition. You can also add a new patient by clicking Add Patientlink on the Patients tab.
3. To view medication logs, click on My Medications/Logs tab and then click on the medication you would like to view logs for (for example, blood pressure medication). The logs will list all of your medication prescriptions and their corresponding dates.
4. If you would like to make a change to any of your patient data, click My Profile/Edit Profile link on the Patients tab and enter your login information. You will then be able to make changes to your profile information, including contact information, medical history and more.

Step 3: On the left-hand side of the page, click on "My Medical Records." This will take you to a list of all of your medical records that are currently being tracked by Meritus.
