To access the patient portal, you first need to create an account. After you create your account, you will be able to login using your email address and password. How to login to Meadville Medical Center Patient Portal
If you are a patient at Meadville Medical Center, you can use their Patient Portal to manage your health records, see your medical bills and read about recent hospital news. To login, follow these steps:
1. Go to the Patient Portal homepage and click on the Login link in the upper-right corner.
2. Enter your patient ID (which is printed on your medical record) and password.
3. Click on the Log In button to finish logging in.
4. You will now be presented with a menu of options for using the Patient Portal. On the left side of the screen, you will see a list of sections that include My Account, Medical Bills and Photos. On the right side of the screen, you will see a list of categories including News & Events, Hospital Policies & Procedures and Social Media.
5. To view or update information in any of these sections, simply click on the corresponding hyperlink. Be sure to remember your patient ID and password so that you can log back in should you need to access your health records or make changes to your account information.

The Patient Portal is a one-stop-shop for information about your hospital stay. You can find out what services and amenities are available to you, register for surgeries and events, and even manage your finances. To login, follow these steps:
1. Click on the "Login" button on the top right corner of the homepage.
2. Enter your username and password in the appropriate fields and click on the "Log In" button.
3. You will be taken to the My Profile page. Here you can access all of your account information, including charts, photos, and notes.
How do I report a problem or concern?
If you're having trouble logging in to the patient portal, or have a concern about your account, please follow these steps:
1. Check to see if you've already contacted Meadville Medical Center through their contact form. If not, please complete the contact form below. They will do their best to resolve your issue as soon as possible.
2. If you've already contacted us and your issue is not resolved, please follow these steps:
- Log in to the patient portal using your user name and password (you may have received this information when you registered for the patient portal).
- Click on "My Account" on the left side of the screen.
- Under "General Information," click on "Contact Us."
- In the "Subject" field, type in your problem or concern and provide as much detail as possible. Please include your full name, email address, phone number and medical record number if available. (Your medical record number can be found on your health history summary page.)
- Click on "Submit."

How can I find my medical records?
If you have registered with Meadville Medical Center's Patient Portal, you can find your medical records by logging in to the Patient Portal. The Patient Portal is accessible through the web browser on your computer, or through a mobile device that has an Internet connection. After logging in, you will be prompted to select a primary care physician from their online provider list. Can I make appointments through the Patient Portal?
Yes, you can make appointments through the Patient Portal. To make an appointment, simply click on the "Appointments" tab and select the date and time that you would like to schedule your appointment. You will then be able to select a doctor and room.
What if I need to speak with a doctor or nurse outside of office hours?
If you need to speak with a doctor or nurse outside of office hours, you can call the hospital's main switchboard at (814) 793-6000 and ask to be connected to the on-call doctor or nurse.
How do I change my password or sign out of the Patient Portal?
If you are a Meadville Medical Center patient and would like to change your password, click the "Login" link in the top right corner of the Patient Portal and follow the instructions. If you have forgotten your login information, click the "Forgot Password" link on the login page and enter your email address in the "Create A Password" field. Once you have entered your email address, you will receive an automated confirmation message containing a link to reset your password. If you have forgotten your password, click on the "Forgot Password" link and enter your new password in the "Create A Password" field. Once you have entered your new password, click on the "Reset Password" button to update your password information.
