How to find information about your health
Intellechart is a patient portal that allows patients to access their health information from anywhere. In order to login to your Intellechart account, you will need your patient ID and password. Here are instructions on how to find your patient ID and password.
How to Find Your Patient ID:
If you have not already done so, go to the My Account page on intellechart.com and create an account. On the My Account page, you will be able to find your patient ID in the upper right hand corner of the screen. The patient ID is a 8 or 12 digit number that begins with an eight and ends with a six.
