The MdHQ Patient Portal is a gateway for patients to access their medical records, medication lists, and more. In this article, we will show you how to login to the portal using your MDHQ credentials.
How to login to the Mdhq Patient Portal
Visit the website https://www.mdhq.gov.uk/patient-portal and click on "Login" in the top right of the screen. Enter your email address and password and click on "Log In". You will then be taken to the main patient portal where you can access your account information, view medical records, make appointments and more.
Finding your account number
If you don't have an account number, you can use your name and date of birth. You can also create an account by clicking on the 'Create Account' button on the main page of the MdHQ Patient Portal. When you create your account, you will need to enter your name and date of birth. You will also need to provide your email address and a password. After you have created your account, you will be able to access it by logging in using your email address and password.
Entering your doctor’s information
Mdhq Patient Portal is a secure online portal that allows you to access your medical records, view your medications and appointments, and speak with your doctor. To login, you will need to provide the following information:
-Your doctor’s name and office address
-Your email address
-Your password
Once you have logged in, you can access all of your account information by clicking on the “My Account” tab. Here, you will find information about your medical history, medications, and appointments. You can also speak with your doctor by phone or video chat using the “Contact Us” tab.

Reviewing your health records
If you have a MyDhq account, you can review your health records from any computer with internet access. To login to your MyDhq account: 1. Go to mydhq.com and sign in. If you don't have a MyDhq account, register for one now. 2. Click the "Health" link in the main menu. 3. On the "My Health Records" page, click the " Login " link in the left column. 4. Enter your user name and password, and click "Log In." 5. You'll see a list of your recent visits and tests in the right column. You can print or download this information as needed.
Requesting copies of your medical records
If you are a patient at theMDHQ Patient Portal, you can request a copy of your medical records. To request a copy of your medical records, follow these steps:
1. Log in to theMDHQ Patient Portal.
2. Click on My Account in the top left corner of the portal.
3. On the My Account page, click on Request a Copy of Your Medical Records.
4. On the Request a Copy of Your Medical Records page, enter your login information and click Submit.
5. A confirmation message will be sent to your email address, and a copy of your medical records will be sent to the email address you provided.
Reviewing and Changing Health Information
If you are new to the MdHQ patient portal, or if you have not been using it for a while, you may want to review how to login and access your health information. The following steps will help you get started:
1. Go to the MdHQ patient portal home page (http://www.michigan.gov/mdhq/portal).
2. Click on 'Login'.
3. Enter your username and password.
4. Click 'Log In'.

Contacting Your Doctor
If you are having trouble locating or logging in to your My Doctor Home Page, follow these simple instructions.
To log in to your My Doctor Home Page:
1. Click on the "My Doctor Home Page" link at the top of any page on the website.
2. Enter your user name and password and click "Login."
3. If you have not set a user name and password, please enter your email address and click "Next."
4. On the next screen, please choose which sections of the website you would like to view.
5. If you have health concerns that require urgent attention, please select "Urgent Attention" and enter your contact information in the space provided.
6. You will then be directed to a secure page where you can leave a voice message for your doctor or request an appointment online.
Requesting Medication History
If you have been diagnosed with a medical condition and would like to request your medication history, the MDHQ Patient Portal is the perfect place to start. You can access the portal by visiting md.gov/patientportal and logging in using your MD account number and password. Once you are logged in, click on the "Medication History" tab at the top of the page. On this page, you will be able to select which medications you would like to request information about. After selecting your medications, click on the "Request Medication History" button. The portal will then request information from your healthcare provider about your medications.
