How to login to the Live Oak Health Partners Patient Portal
If you are a Live Oak Health Partners patient, you can access your patient portal by logging in with your login credentials. To access your patient portal, go to the Patient Portal section of their website and enter your Patient ID (found on your medical record) into the login form. After entering your login information, you will be able to view all of your health records and receive notification of doctor appointments and other important updates from their team.
How to find your account number
If you have an account with Live Oak Health Partners, you can find your account number by following these steps:
1. Log in to the Patient Portal.
2. On the left-hand side of the screen, under "My Account," click on "Account Numbers."
3. On the next page, under "Account Numbers," click on your account number to view your account information.
How to change your password
If you have forgotten your Live Oak Health Partners password, you can change it online. To change your password, log in to the Patient Portal at www.liveoakhealthpartners.org and click on the “My Account” tab. On the My Account page, click on the “Password” link in the login column. Enter your current password in the “New Password” box and type a new password in the “New Password (again)” box. Click on the “Change Password” button to save your changes.
How to view your medical records
If you have enrolled in Live Oak Health Partners, you can view your medical records by logging in to their Patient Portal. To access the Patient Portal, go to www.liveoakhealthpartners.com and sign in using your login credentials. Once you are logged in, click on "My Account" on the left-hand side of the screen. From there, select "View Medical Records." You will be prompted to choose which type of record you would like to view: Medical History, Medications, or Tests. Once you have selected the record you want to view, you will be able to view all of the information contained within that record.
