How to make a request for a change in your treatment plan
If you would like to make a change in your treatment plan, please follow these instructions:
Login to the Kingwood Psychiatry Patient Portal. Click on the "My Account" link in the header of the homepage. Click on "Treatment Plans" from the menu on the left. Click on "Request a Change in Treatment Plan" from the drop-down menu next to your treatment plan. In the "Supplemental Information" box, please provide information about why you want to change your treatment plan and what you hope to achieve by changing it. If you are requesting a change for more than one reason, please list them all in this box. In the "Additional Comments" box, please explain any additional information or reasons that you feel should be included in your treatment plan request. Once you have completed all of the required fields, click on the "Request a Change in Treatment Plan" button. Your therapist will contact you to discuss your request and answer any questions that you may have.
