How to find your Jphsa User ID and Password
To login to your Jphsa patient portal, you will need to find your user ID and password. Your user ID is located on the upper left-hand corner of your login screen and your password is located in the password field. How to update your Jphsa User ID and Password
To update your Jphsa User ID or Password, please follow these steps:
1. Open the Jphsa Patient Portal and sign in.
2. Click on "My Profile" in the top left corner of the screen.
3. In the "Profile Details" section, click on "Update My Profile."
4. Enter your User ID and Password and click on "Update Profile."

How to change your email address
If you have ever changed your email address while logged in to the Patient Portal, you will need to update your login information as well. To do this:
1. From the main Patient Portal page, select "My Profile" on the left-hand side.
2. On the My Profile page, scroll down and click on "Personal Info".
3. In the Email Address field, type your new email address and then click on the "Change" button.
4. Click on the blue "Update My Profile" button to finish updating your information.
How to remove your account from Jphsa Patient Portal
If you no longer need access to the Jphsa Patient Portal, you can remove your account by following these steps:
Sign in to your account at www.jphsa.org/patientportal Select "My Account" on the main menu In the "Profile" section, click on "Privacy & Security" On the "Remove My Account" page, click on "Remove My Account"
